At that check-up, ask your dentist to stamp or sign the form.

Purchase a Colgate toothpaste or toothbrush.

Send the form (along with one proof of purchase from a Colgate toothpaste or toothbrush) to Colgate and we'll send you a
free battery-powered tootbrush!
